The my mhealth Digital Transformation Team has been working with providers, services and CCGs assisting their transformation from current service delivery, to digitally enabled patient pathways. Our clinicians work with local teams and help teams appraise their current pathways to identify where the Apps are best distributed and implemented. Our experienced engagement specialists then assist with the deployment.
Digital transformation takes place over 3 stages of services:
• System activation – To make sure the provider is able to deploy the solution as part of the current IT set up.
• Clinician activation – Enabling and training clinical teams.
• Patient activation – This final stage is where the service onboards and supports the patient.
