When you are in the patient profile you will notice the ‘manage my patient’ button at the top of the screen. Selecting this button will bring up a menu of features to support your clinical review and management of your patient, including:
Details
- Review and update patient details
Reports
- View patient results over time from the screening assessments
Video consultation
- Schedule 2-way video consultations with patient – appointments will appear as a notification in the patients app as well as sending an email diary invite
- Join scheduled video consultations
- View recordings of previous consultations
One way messaging
- Send messages to individual patients with the ability to include links to external resources – patients cannot respond to these messages from within the app
Activity diary
- View reports of how active your patients have been with data collected from in app activity automatically, as well as manually entries from the patient and those connected via activity trackers
Patient activation
- See if your patient has fully activated their account, check their email address and amend if required and resend the activation message if needed
Remove patient
- Here you can remove the patient from your caseload by selecting the most suitable option:

You will also see the other long term condition app tiles on this menu. If your patient has one or more of these long-term conditions and has access to the my mhealth app to support them, you can navigate to the disease specific caseload to support holistic management of your patient.
